Our Programs

 

Access Partnership's focus is to strengthen the health care safety net systems of care in our communities by expanding existing programs, implementing new services for identified gaps in primary care and chronic illness prevention and coordinating care for medically indigent families.

 

Community Access to Care Program

This is a coordinated care program that provides access for uninsured patients between safety net providers and diagnostic/specialty care services. The process increases the equitable and seamless provision of free or reduced care among an existing network of primary care safety net providers thereby improving health of low income, uninsured adults in Greater Hampton Roads.

What is Access Partnership's Community Access to Care Program?

 

The program provides access to diagnostic and specialty care for free or for a reasonable administrative fee to financially eligible patients who receive primary care at a partnering community health center or clinic.

 

How do I enroll?

 

This is not a self referral program. Qualified persons are invited to participate based on eligibility criteria (see below). The program is currently limited to 1000 enrollees.

What are the qualifications for enrollment?

 

·        Established patients of Hampton Roads Community Health Center (Norfolk site only), Main Street Physicians (Suffolk), Sentara Ambulatory Care Clinic (Norfolk) or the Virginia Beach Family Medical Center

·        Between the ages of 19 and 64 years of age

·        At or below 200% of the federal poverty guidelines (see below)

·        Do not have or qualify for any health insurance coverage

·        Do not have or qualify for federal, state or local health benefits (i.e. Commercial, Medicare, Medicaid, Veteran's Benefits, FAMIS)

 

Which health care providers are participating in the program?

 

Currently, Sentara Hospitals and outpatient centers are the health care facilities participating in the program. Hospital based physician services from the following specialties also participate:

·         Anesthesia

·        Radiology

·        Pathology 

 

What's next?

 

Outcomes are continually reviewed to identify program efficiency and opportunities for improvement. Our goal is to expand the program to include more patients and other primary care sites in South Hampton Roads* in the future.

Federal Poverty Guidelines

 

The poverty guidelines are issued each year in the Federal Register by the Department of Health and Human Services (HHS). The guidelines are for use for administrative purposes for instance, determining financial eligibility for certain federal programs.
